What happened:

Backloaded with lack of information. Sent to wrong vendor/handled wrong. The casing was mistakenly thought to have been in hole and delivered to Wellcon as per previous Oselvar operations. Should have been Mitsui, however no indication of this was in backload manifest.

What was planned:

Backload of casing joints not run in hole.

What was learned:

Higher focus on quality of backload by marking what type of equipment is to go where. This is to avoid “habits”, as by sending it to Wellcon as previous operation.

What actions are required:

Look into procedures for better highlighting in future. Review manifest names on backoad to highlight well/project.
